Synopsis: The benefit of mechanical thrombectomy in acute ischemic stroke due to large vessel intracranial occlusions is directly related to the technical success of the procedures in achieving a fast and complete reperfusion.
The main technique popularly practiced from second half of 2000s was a stent retriever thrombectomy with proximal flow arrest and manual aspiration using a balloon-guide catheter. Aspiration was done too far from the thrombus and achieving favorable reperfusion often required multiple thrombectomy attempts and sometimes the use of two different stent retrievers.
Nowadays the contact aspiration with a vacuum pump is a main tool in many centers.
In my presentation I’ll discuss the pros and cons of Sofia Non-wire Advancement technique (SNAKE) followed by A Direct Aspiration first Pass Technique (ADAPT) which is mostly used in our center.
