Synopsis: We discuss the basics of PAE. From how many centres are offering the service and the relative costs involved to the basics needed to deliver the service. Through cross sectional planning to basic anatomy and equipment required we discuss the controversies around PAE. Who, how and why? Which patients are ideal candidates and how to select the most appropriate cases when starting your service. We finally discuss some of the less common but concerning complications and controversies around morphology and a negative CT
