Synopsis: Osteoarthritis is a chronic degenerative-inflammatory joint disease and one of the major causes of disability worldwide. Classic radiological signs such as joint space narrowing, cartilage loss, osteophyte formation and others are widely recognized. However, modern investigations are focused on pathogenesis more than consecutive changes. Low-grade inflammation, especially synovitis, which was latterly recognized as a substantial factor in OA pathogenesis, plays an important role in OA development and is associated with pain and further cartilage degeneration. Synovial evaluation and quantification with non-invasive imaging tools is becoming increasingly important both in daily clinical practice and research. Various ultrasound techniques, non-contrast MRI and contrast enhanced MRI are used on daily basis. More complex and innovative methods such as dynamic contrast-enhanced MRI and molecular imaging techniques are being investigated in synovitis diagnostics with promising results.
