Synopsis: Hydatid disease or echinococcosis is a worldwide parasitic disease, that can occur almost anywhere in the body. The lung parenchyma is the second most affected organ after the liver, but it may develop in the pleural cavity, mediastinum, heart, vascular structures, chest wall, and diaphragm. Diagnosis is mainly based on serological and radiological findings. Radiologic findings can be very different and vary from purely cystic lesions to a completely solid appearance, it depends on type of infection, growth stage, affected tissue and associated complications. So, radiologists must be familiar with the imaging features of this disease and contemplate the diagnosis when facing atypical chest lesions, especially in patients whose work involves farm animals like sheep’s or their breeding or with evidence of echinococcosis in a different anatomical location.
